ACKNOWLEDGEMENTS & RELEASE FORM

1. I UNDERSTAND THAT I AM SIGNING UP FOR A VOLUNTEER ROLE I acknowledge that I am volunteering my time and efforts for Christ through the ministries of Revival in the Nations and I will receive no monetary compensation or insurance benefits for my time or services performed. 2. I AM VOLUNTEERING AT MY OWN RISK I acknowledge and accept the risk of physical injury that could occur from my participation as a volunteer. I accept personal financial responsibility for any bodily or personal injury sustained while I am volunteering. Furthermore, I release and agree to indemnify and hold harmless Revival in the Nations and its leaders and volunteers from any liability, injury, damage, loss, accidents, delay or irregularity related to being a volunteer for Revival in the Nations. 3. YOU CAN TAKE MY PICTURE I grant to Revival in the Nations the right to make photo, video, and audio recordings of me as well as the right to publicly display and distribute the recordings in any way consistent with Revival in the Nation’s ministry, including use on the internet, website and social media. I waive the right to inspect or approve the finished product wherein the photo or recording appears. EMERGENCY AUTHORIZATION

In the event of an emergency, and if I (Parent/Legal Guardian) cannot be reached, I hereby authorize the adult volunteer in charge to seek any medical treatment recommended by a qualified nurse, doctor, or hospital for my son/daughter. Parent(s) or court-appointed legal guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms & conditions of this document, as set forth above as acknowledged by the signature below.
