  • Bring a Packed Lunch

  • Bring a backpack for carrying event flyers

  • Wear layers and comfortable shoes (prepare for the weather / outdoor street mission)

Terms and Conditions RE: AGE & FITNESS

*MINIMUM AGE 14 YEARS
To be accompanied by a parent/carer/leader with (parental permission signed waiver)

*16-18 YEARS (parental permission signed waiver is required)

*To participate in the street mission outreach, you must be physically able to walk at a medium to fast pace for 2 hrs.

ADULT WAIVER

This Adult Waiver Form is required if you have signed up to volunteer in any capacity with Revival In The Nations.

The signing of this waiver is valid from the date of signing for all current and future involvement (there is no end date) 

To participate in the (STREET) Mission Outreach during Revival Camp, you MUST agree and sign the Adult Waiver Form.

MINOR WAIVER

This Waiver Form is required if a Minor (under 18 years) has signed up to volunteer in any capacity with Revival In The Nations. 

The signing of this waiver must be completed by a Parent/legal guardian and will be valid from the date of signature until the Minor is 18 years of age. If this form is signed by a church leader it will be valid for the duration of the applicable event.
